That’s not a guideline to bend. It’s a hard stop, and any legitimate prescriber will ask about it directly.

Beyond that, conditions like diabetes, high blood pressure, and prostate issues don’t necessarily rule anything out, but they do affect which dose makes sense and how quickly you should expect results. Men with diabetes sometimes find that ED medications work less reliably, or need a higher dose to get the same effect. That’s not a failure; it’s just biology, and it’s worth having a conversation with a doctor about rather than quietly increasing your own dose because the standard one didn’t seem to do much.

There is another layer that a medication checklist cannot capture. Treatment may improve the physical response while nerves still linger. For some men, the improvement breaks that loop; others prefer to combine medical help with a conversation about anxiety. Either route can make sense.

One question worth taking along

A lot of men go into their first consultation with a specific medication already in mind and treat the appointment as a formality. I can see why, but it can narrow the conversation too soon. I would ask the prescriber to weigh up my age, current medicines and wider health, then explain which option they would try first and the reason for it. That gives the appointment somewhere useful to go.

The medication is almost the simple bit. The rest takes a bit more thought.
